Registration
In most cases you need to
arrive approximately
one-and-half to
two hours prior
to your scheduled
procedure time. This is required
for the registration process
and
to prepare you for surgery.
During registration we will verify your name, date
of birth, social security number, address and insurance information. At the time
of registration, an armband with your correct identification will be placed on your
wrist that will remain with you until discharge.
Items to Bring
Before you leave your house, please remember to bring the following items, if
applicable:
- Insurance information,
- All home medications in the original containers if you will be staying
overnight in the hospital,
- An updated list of all medications you are currently taking if you are having outpatient surgery,
- Any assistive devices you may need such as crutches, walker, brace or any device the Physician has ordered for you.
- Any personal hygiene items and change of clothes.
Also, please remember to remove all jewelry including rings and body piercing. If you are having surgery on the hand, wrist or shoulder, extreme measures such as manually cutting off the ring may be necessary to prevent potential complications from swelling.
